Information

Completed registration form for new student.
Submit your registration fee of $250 per family.
A fundraising fee of $350 per family is divided over 12 months through FACTS. Participation in the Rewards Program will allow a family to earn $300 back as tuition credit.

Forms must be completed and returned on or before the First Day of School in August. **

ADDITIONAL FORMS ONLY NEED TO BE COMPLETED WHEN APPLICABLE:

Illinois Department of Public Health Certificate of Child Health Examination, Dental Examination Record, and Illinois Eye Examination Report forms completed by your child’s physician, dentist, and optometrist are all students entering Kindergarten in the 2023-2024 school year. Current immunizations records must be on file for all students entering the Jaguar Cubs and Junior Kindergarten classes. A health examination form and dental examination are required for those entering 6th Grade in the 2023-2024 year. Dentals forms are required for students entering 2nd grade in the 2023-2024 year. These forms or written notice of upcoming appointments must be completed the first day of school.
